Setting the scene
During the Spring Plenary in 2018, integrated care has been mentioned as a topic to be pursued. Along with the MOCHA project and a workshop on integrated care organised within the Artic Light e-Health Conference, AER just joined a partnership for a new project on this topic, thus putting into action a recent decision taken by Committee 2.
Description of the action
The SCIROCCO Exchange project, under the 3rd Health Programme, is based on the SCIROCCO project, which has resulted in the development of an online self-assessment tool for integrated care, as well a on the work of the B3 Action Group of the EIP on AHA.
The SCIROCCO Exchange project aims at supporting regions, mainly health and social care authorities for the adoption and the scaling-up of integrated care. This will be realised by facilitating their access to evidence-based assets on integrated care, as well as by encouraging personalised knowledge transfer and improvement planning on this topic.
The main result will be a better access to evidence on integrated care and tools available to potential adopters, as well as a better understanding of the readiness of regions for the adoption of integrated care.
Within this project, AER will lead work package dealing with knowledge transfer. Two mains objectives are targeted: supporting regions to tailor the local needs and priorities and improving their existing system. This will be done through different actions. On one side, will be designed personalised assistance and practical support in order to tailor the needs of the regions and their priorities and to improve their existing health system. On the other side, AER will facilitate knowledge transfer between health and social care authorities from different regions across Europe.
Why is this action important for regions?
This project is a warm welcome to one of our new members National Health Services Scotland, who is the lead partner in this project. But most of all, integrated care is a key issue for regions. The SCIROCCO Exchange project will allow regions to obtain information and resources to better adopt and implement integrated care.
